No one is finger pointing or dung slinging.
I think it is more people are interested in what the issue was, why and what is already in place.
The assumption is that when something is broken (a process) it will get fixed by knowledgible people. To add a level of bureaucracy is probably not needed after one occurance. Just an understanding of what happen and who should have done what.
When a problem becomes systemic is when you need to add additional bureaucracy.

when a problem is systemitic the last thing you would need is additional levels of bureaucracy. When a problem becomes systemic, it means the system in place is wrong and repeats the problem over and over, not just one person. In which case you need to change the system or add a watch dog.
