Please define: helmet to helmet/targeting

Please define: helmet to helmet/targeting
At least three times today, sac defenders lowered their heads and lead with the crown of the helmet while making tackles.
1) On L Jones long run play, he got lit up at the line of scrimmage. Kept his balance and made a long gain, all while trying to adjust his helmet. Replay clearly shows the CB coming down the LOS and leads with the crown and clearly hits Jones in the head. It was audibly, clearly a helmet to helmet whack!
2) Very next play, our receiver is in the left flat being spun around by the initial defender when he gets walloped with the crown of the helmet hit.
3) J Allyenes punt return on the sac sideline that knocked him out of the game. Crown of the helmet hit.
What am I missing, other than we know BSC referees suck whale sh!t?

We'll get that answered once we also get answered why a play that produced a TD was called off because the refs called the play dead for too many men on the field of play.

Pretty sure that was not the penalty, it was for illegal substitution, which would be a dead ball penalty. When Team A sends in its substitutes, the officials will not allow the ball
to be snapped until Team B has been given an opportunity to substitute.
While in the process of substitution or simulated substitution, Team A
is prohibited from rushing quickly to the line of scrimmage with the
obvious attempt of creating a defensive disadvantage. If the ball is ready
for play, the game officials will not permit the ball to be snapped until
Team B has placed substitutes in position and replaced players have left
the field of play. Team B must react promptly with its substitutes.
PENALTY—(First Offense)—Dead-ball foul. Delay of game on Team B
for not completing its substitutions promptly, or delay of

notify the head coach that any further use of this tactic will
result in an unsportsmanlike conduct foul.

I'm just confused on when they decided to call that the foul instead of offsides or 12 men on the field because I don't recall the play being blown dead until its conclusion. I did not get to see a replay though and couldn't hear what the call was because the mike was out or something. #3 The hit against Alleyne too was also out of bounds, but I didn't have the best angle. Not nearly as close as the Ref on that side. What I did see is the Sac St player fly in, right as/after the whistle with his head lowered.
